    OPTIMIZATION OF STATION LOCATIONS AND TRACK ALIGNMENTS FOR RAIL TRANSIT LINES

    Designing urban rail transit systems is a complex problem, which involves the determination of station locations, track geometry, right-of-way type, and various other system characteristics. The existing studies overlook the complex interactions between railway alignments and station locations in a practical design process. This study proposes a comprehensive methodology that helps transit planners to concurrently optimize station locations and track alignments for an urban rail transit line. The modeling framework resolves the essential trade-off between an economically efficient system with low initial and operation cost and an effective system that provides convenient service for the public. The proposed method accounts for various geometric requirements and real-world design constraints for track alignment and stations plans. This method integrates a genetic algorithm (GA) for optimization with comprehensive evaluation of various important measures of effectiveness based on processing Geographical Information System (GIS) data. The base model designs the track alignment through a sequence of preset stations. Detailed assumptions and formulations are presented for geometric requirements, design constraints, and evaluation criteria. Three extensions of the base model are proposed. The first extension explicitly incorporates vehicle dynamics in the design of track alignments, with the objective of better balancing the initial construction cost with the operation and user costs recurring throughout the system's life cycle. In the second extension, an integrated optimization model of rail transit station locations and track alignment is formulated for situations in which the locations of major stations are not preset. The concurrent optimization model searches through additional decision variables for station locations and station types, estimate rail transit demand, and incorporates demand and station cost in the evaluation framework. The third extension considers the existing road network when selecting sections of the alignment. Special algorithms are developed to allow the optimized alignment to take advantage of links in an existing network for construction cost reduction, and to account for disturbances of roadway traffic at highway/rail crossings. Numerical results show that these extensions have significantly enhanced the applicability of the proposed optimization methodology in concurrently selecting rail transit station locations and generating track alignment

    The Chinese version of the Health Professional Communication Skills Scale: Psychometric evaluation

    ObjectiveThis study aims to translate the Health Professional Communication Skills Scale (HP-CSS) into Chinese and assess its psychometric properties.MethodsA total of 836 healthcare professionals were recruited. The demographic characteristics form and HP-CSS were used for data collection. The psychometric properties of HP-CSS were evaluated by examining item analysis, construct validity, known-group discriminant validity, internal consistency, and split-half reliability.ResultsIn terms of item analysis, the critical ratio (CR) of 18 items was both >3 (CR ranging from 9.937 to 28.816), and the score of each item was positively correlated with the total score (r ranging from 0.357 to 0.778, P < 0.001). The fit indices showed that the original correlated four-factor model of HP-CSS was adequate: χ2 =722.801; df = 126; χ2/df = 5.737; RMSEA = 0.075; CFI = 0.923; NNFI = 0.908; TLI = 0.906; IFI = 0.923. In terms of known-group discriminant validity, the HP-CSS total score was related to gender, occupation, work years, and communication skill training. Cronbach's α coefficient was 0.922, and the split-half reliability was 0.865 for the total scale.ConclusionThe Chinese version of the HP-CSS is a reliable and valid instrument to evaluate communication skills among healthcare professionals in China

    Surface-Associated Plasminogen Binding of Cryptococcus neoformans Promotes Extracellular Matrix Invasion

    BACKGROUND:The fungal pathogen Cryptococcus neoformans is a leading cause of illness and death in persons with predisposing factors, including: malignancies, solid organ transplants, and corticosteroid use. C. neoformans is ubiquitous in the environment and enters into the lungs via inhalation, where it can disseminate through the bloodstream and penetrate the central nervous system (CNS), resulting in a difficult to treat and often-fatal infection of the brain, called meningoencephalitis. Plasminogen is a highly abundant protein found in the plasma component of blood and is necessary for the degradation of fibrin, collagen, and other structural components of tissues. This fibrinolytic system is utilized by cancer cells during metastasis and several pathogenic species of bacteria have been found to manipulate the host plasminogen system to facilitate invasion of tissues during infection by modifying the activation of this process through the binding of plasminogen at their surface. METHODOLOGY:The invasion of the brain and the central nervous system by penetration of the protective blood-brain barrier is a prerequisite to the establishment of meningoencephalitis by the opportunistic fungal pathogen C. neoformans. In this study, we examined the ability of C. neoformans to subvert the host plasminogen system to facilitate tissue barrier invasion. Through a combination of biochemical, cell biology, and proteomic approaches, we have shown that C. neoformans utilizes the host plasminogen system to cross tissue barriers, providing support for the hypothesis that plasminogen-binding may contribute to the invasion of the blood-brain barrier by penetration of the brain endothelial cells and underlying matrix. In addition, we have identified the cell wall-associated proteins that serve as plasminogen receptors and characterized both the plasminogen-binding and plasmin-activation potential for this significant human pathogen. CONCLUSIONS:The results of this study provide evidence for the cooperative role of multiple virulence determinants in C. neoformans pathogenesis and suggest new avenues for the development of anti-infective agents in the prevention of fungal tissue invasion

    Global, regional, and national incidence and mortality for HIV, tuberculosis, and malaria during 1990–2013: a systematic analysis for the Global Burden of Disease Study 2013

    BACKGROUND: The Millennium Declaration in 2000 brought special global attention to HIV, tuberculosis, and malaria through the formulation of Millennium Development Goal (MDG) 6. The Global Burden of Disease 2013 study provides a consistent and comprehensive approach to disease estimation for between 1990 and 2013, and an opportunity to assess whether accelerated progress has occured since the Millennium Declaration. METHODS: To estimate incidence and mortality for HIV, we used the UNAIDS Spectrum model appropriately modified based on a systematic review of available studies of mortality with and without antiretroviral therapy (ART). For concentrated epidemics, we calibrated Spectrum models to fit vital registration data corrected for misclassification of HIV deaths. In generalised epidemics, we minimised a loss function to select epidemic curves most consistent with prevalence data and demographic data for all-cause mortality. We analysed counterfactual scenarios for HIV to assess years of life saved through prevention of mother-to-child transmission (PMTCT) and ART. For tuberculosis, we analysed vital registration and verbal autopsy data to estimate mortality using cause of death ensemble modelling. We analysed data for corrected case-notifications, expert opinions on the case-detection rate, prevalence surveys, and estimated cause-specific mortality using Bayesian meta-regression to generate consistent trends in all parameters. We analysed malaria mortality and incidence using an updated cause of death database, a systematic analysis of verbal autopsy validation studies for malaria, and recent studies (2010-13) of incidence, drug resistance, and coverage of insecticide-treated bednets. FINDINGS: Globally in 2013, there were 1·8 million new HIV infections (95% uncertainty interval 1·7 million to 2·1 million), 29·2 million prevalent HIV cases (28·1 to 31·7), and 1·3 million HIV deaths (1·3 to 1·5). At the peak of the epidemic in 2005, HIV caused 1·7 million deaths (1·6 million to 1·9 million). Concentrated epidemics in Latin America and eastern Europe are substantially smaller than previously estimated. Through interventions including PMTCT and ART, 19·1 million life-years (16·6 million to 21·5 million) have been saved, 70·3% (65·4 to 76·1) in developing countries. From 2000 to 2011, the ratio of development assistance for health for HIV to years of life saved through intervention was US$4498 in developing countries. Including in HIV-positive individuals, all-form tuberculosis incidence was 7·5 million (7·4 million to 7·7 million), prevalence was 11·9 million (11·6 million to 12·2 million), and number of deaths was 1·4 million (1·3 million to 1·5 million) in 2013. In the same year and in only individuals who were HIV-negative, all-form tuberculosis incidence was 7·1 million (6·9 million to 7·3 million), prevalence was 11·2 million (10·8 million to 11·6 million), and number of deaths was 1·3 million (1·2 million to 1·4 million). Annualised rates of change (ARC) for incidence, prevalence, and death became negative after 2000. Tuberculosis in HIV-negative individuals disproportionately occurs in men and boys (versus women and girls); 64·0% of cases (63·6 to 64·3) and 64·7% of deaths (60·8 to 70·3). Globally, malaria cases and deaths grew rapidly from 1990 reaching a peak of 232 million cases (143 million to 387 million) in 2003 and 1·2 million deaths (1·1 million to 1·4 million) in 2004. Since 2004, child deaths from malaria in sub-Saharan Africa have decreased by 31·5% (15·7 to 44·1). Outside of Africa, malaria mortality has been steadily decreasing since 1990. INTERPRETATION: Our estimates of the number of people living with HIV are 18·7% smaller than UNAIDS's estimates in 2012. The number of people living with malaria is larger than estimated by WHO. The number of people living with HIV, tuberculosis, or malaria have all decreased since 2000. At the global level, upward trends for malaria and HIV deaths have been reversed and declines in tuberculosis deaths have accelerated. 101 countries (74 of which are developing) still have increasing HIV incidence.     Optimization of Rail Transit Alignments Considering Vehicle Dynamics

    Urban rail transit systems are being extended throughout the world because of their large capacities, avoidance of traffic congestion, and environmental advantages. Various optimization models can help design rail transit alignments satisfying various track geometry constraints, but none of these models can account for the impacts of vehicle dynamics on operational and user costs. This paper presents a practical rail transit alignment optimization method for designing track alignments that accounts for vehicle dynamics. The method can generate alignments that improve the balance between the initial cost and the operation and user costs recurring throughout the system\u27s life cycle. A heuristic based on a genetic algorithm is developed to search for solutions efficiently while interacting with the supporting geographic information system. A hypothetical topography scenario is created to illustrate the impact of vehicle dynamics on the trade-offs among system costs. The Baltimore, Maryland, Red Line is used as a case study to demonstrate that the model can find good solutions in regions with complex topographies

    Corridor-Based Emergency Evacuation System for Washington, D.C.: System Development and Case Study

    The evacuation of large municipal areas in an efficient manner during emergencies and disasters is one of the critical tasks of emergency management agencies. This paper presents a corridor-based emergency evacuation system and an example application of the system for the Washington, D.C., metropolitan area. The proposed system features flexibility by accounting for various critical issues associated with both planning and real-time operations, including the integration of data from multiple sources, network decomposition, network-level traffic routing, contraflow design, staged evacuation, optimal signal timing, and the incorporation of pedestrian and bus operations. Under a hypothetical emergency scenario for Union Station in Washington, D.C., the proposed system demonstrated its effectiveness at producing evacuation routing strategies, identifying potential bottlenecks, and evaluating the performance of evacuation operations

    The construction of expressway and high-speed rail is at the sacrifice of the soil ecological environment. It brings about much damaged land and bare slope. It is necessary to restore vegetation and rebuild landscape. In the design of target plant community and configuration plants, it is required to select as far as possible plants that can blossom and bear fruit or pods. Then seeds of these plants can be harvested, processed and sold, and applied for other projects, so as to develop into a complete forest and grass seed industry chain. This not only reasonably utilizes land resources, but also provides a new approach to the problem of insufficient fine forest seed sources in China. This paper discussed the use of land resources in road side slope and forest and grass seed selection and breeding technology, and elaborated the industrialized development paths for forest and grass seed industries in road side slope. Finally, it arrived at following recommendations. (i) The construction of forest and grass industry economic belt using land resources in road side slope can realize maximum land value. Building a road can help people there get rich, and the type of land and water cultivates its type of forest and grass. (ii) It can make up for the losses incurred from construction of expressway and high-speed rail or destroying forest land, and it can increase farmers’ income. (iii) It can increase the supply of domestic seeds, reduce seed import, save foreign exchange, and partly solve the problem of insufficient supply of forest and grass seeds. (iv) It is able to form a complete forest and grass seed industry economic cycle chain, increase employment, and provide new approaches for enriching and benefiting farmers
